Capture Form Field - Zip Code

I need to be able to collect a Name, Email, and
Zip Code, then have that data sent to a corresponding Shopify customer
profile. I am searching far and wide for a conversion marketing solution
that will do that.

At the moment, we only have Name, Email, Phone and Birthday information. Would be great if we can have an additional field to collect Zip code information or for adding some additional note
